SPS is celebrating Safety Week on March 9-15! In celebration of #SPSsafetyweek2025, we’re highlighting the awesome patient and employee safety improvement work within our organizations! Next up is Children's of Mississippi.

A Story of Vigilance and Care
The clinics at Children's of Mississippi administer vaccinations to patients daily, making a well-stocked fridge of vaccines essential. Recently, during her routine checks, Erin Windham noticed something amiss—one less diluent than expected. Knowing that the diluent should always be paired, Erin's concern was immediate. She began investigating, reviewing patient charts from the previous day.
Erin's diligence paid off. She discovered that a novice nurse had inadvertently administered only the diluent instead of the full vaccine. Acting swiftly, Erin informed her supervisor, Lee Harbour, who then reached out to the affected patient's family.
A Heartfelt Response
Lee prepared for a range of reactions from the caregivers, but to his surprise, the patient's mother responded with gratitude. She thanked Lee and the team for their honesty and care, praising Erin for her vigilance and the entire team for fostering a culture of safety.
Implementing Change
In response to this incident, a new process was implemented to prevent similar mistakes in the future. The diluent is now stored separately, ensuring that such an error does not occur again.
