SPS is celebrating Safety Week on March 9-15! In celebration of #SPSsafetyweek2025, we’re highlighting the awesome patient and employee safety improvement work within our organizations! First up is Holland Bloorview Kids Rehabilitation Hospital!
Holland Bloorview Safety Coach Team
At Holland Bloorview Kids Rehabilitation Hospital, safety is a top priority. A dedicated team of 23 safety coaches, consisting of both clinical and non-clinical staff, plays a crucial role. These coaches work tirelessly across both ambulatory and inpatient settings to ensure a safe environment for everyone.
Engaging Safety Coach Meetings
Every other month, the hospital hosts well-attended safety coach meetings. These gatherings have become a key space for collaboration and sharing insights, with staff actively engaged and eager to contribute.
Spotlighting Safety Coaches
Since September 2024, the hospital has been profiling one safety coach each month on the intranet, “HB Connect.” These profiles highlight the coaches' work and expertise on various topics that contribute to a culture of safety. Topics covered include advocating for an error-free environment, identifying gaps in the pediatric rehabilitation system, working towards zero harm, and fostering a culture of safety through error prevention practices.
Comprehensive Error Prevention Training
The hospital launched an error prevention training program that has now been completed by over 85% of the staff. This training is also a requirement for all new staff, who complete it through e-learning modules. To further support the hospital’s safety culture, the training was expanded to include contracted staff, such as those in environmental and food services, and made available in multiple languages to ensure accessibility.
Innovative Use of the Behavior Observation Tool (BOT)
One impactful initiative has been the use of the Behavior Observation Tool (BOT). This tool allows safety coaches to track and capture the uptake of various error prevention tools, providing valuable insight into where these tools are being used and how effectively they are being integrated into practice. This ongoing effort is helping to build a safer environment for clients, their families, and staff, ensuring that everyone is “Caring Safely. Always.”
